Narrative:
The JAT flight took off from Belgrade at 12:30 GMT for a flight to Vienna. At 14:25 the flight contacted Vienna Approach Control. Five minutes later the flight reported over the OEW beacon and was instructed to remain 500 feet above the clouds. A little later the flight was cleared for a runway 12 instrument approach and to descend to 3500 feet. At 14:40 the aircraft crashed on the uphill slope of the Kahlenberg hill.
PROBABLE CAUSE: "The aircraft flew for a longer period on the outbound track and descended below the prescribed minimum altitude laid down at the time by the airline for operations into Schwechat aerodrome." CREW ERROR - Navigation error, descended below minimums.

Narrative:
The Tu-134 approached Rijeka (RJK) in poor weather with heavy cumulo-nimbus, heavy rainfall and severe turbulence about 4km from the runway threshold. As the aircraft passed through the area, it was carried upwards and rolled to the right. The aircraft was now above the ILS glide path. Some 800m short of the runway, at a height of 60m the crew decided to continue the approach. Power was reduced and the angle of descent increased to 10 degrees. The aircraft struck the runway with the right main gear at an airspeed of 160km/h with 4g deceleration force. The right wing detached and the plane rolled on its back. The remaining stub of wing folded to block the overwing emergency exit. The aircraft slid for 700m and caught fire.

Narrative:
The aircraft was on a flight from Ljubljana to Ajaccio and entered a holding pattern. The aircraft descended through the minimum holding altitude of 6800 feet and collided near the top of the Mt. San-Pietro (4477 feet high).
Factor in the accident was the imprecise language used by the DC-9 crew and the air traffic controller. The DC-9 was flying a holding pattern to lose altitude, but the controller believed the aircraft was going to make a direct descent to begin the final approach.
After several aural GPWS warnings (starting 13 seconds before impact) the crew increased engine thrust relatively slowly and pulled up too late to prevent the accident.
